Shawna lives in Arizona and was ordained by American Marriage Ministries on February 26th, 2017.
I'm a licensed mobile notary and wanted to expand my services by offering to perform wedding ceremonies as well. I believe that if you love someone and want to spend your life devoted to that one person, then I would provide that service at any hour of the day 24/7. So, along with my 24/7 Mobile Notary service, I'm offering wedding ceremonies to my list of services I provide. You will need to provide a marriage license obtained at any superior court location in Arizona and they are valid for 1 year after the Bride to be and Groom to be BOTH together are required to obtain the license in Arizona and swear an oath of facts and identity verification to the courts; once that license is obtained, I can perform the marriage ceremony. I provide a beautiful ceremony certificate for you to keep to commemorate your union of marriage and I certify your marriage license after the ceremony is performed; and I will submit the license with my signature that your ceremony was performed, to the courts, so they may record the marriage. You should receive a certified copy of the recording (if you elected that option, at the time you obtained your license at the courthouse), within 30 days. You will need that certified document if the Bride intends on changing her name at the DMV and Social Security Office. Both changes require you to appear in person. Step 1.) The Bride must go to the Social Security Office first. Bring certified copy of the recorded marriage once it's been mailed to you; along with valid photo ID; old SS card; Certified Copy only of Birth Certificate. You will then hand those documents so they can update your former last name to new marriage name. Step 2.) Since the DMV only updates their information to match the information shared with the SS administration, they can only change your name AFTER you submit name change to SS Admin. Visit your local DMV office and take your current DL; new SS Card that shows your New Last Married Name; and certified copy of the Marriage License so they may update your name, address and photo. There is a cost involved in most cases, of obtaining new DL, so bring $ with you. Contact your local MVD office to find out the cost. Step 3.) Contact your bank; Insurance Agent; Credit Card companies; and 3 Credit Bureau Agencies of new name change. They will require evidence of the name change, so this is why you should request to have Certified Copy of Marriage License mailed to you from the Court. There is no cost to change your name with them, unless your bank needs to reorder checks with your new name. If you have other questions please feel free to contact me.
